3. Core Scope & Key Content Highlights (English)

 
This is China’s first dedicated clinical practice guideline for elderly hypertension, covering:
 
  • Epidemiology & Burden: Hypertension prevalence >50% in adults ≥60 years, ~90% in those ≥80 years; low control rate (18.2% in 2015 vs. 7.6% in 2002).
  • Diagnosis & BP Measurement: Office, home (HBPM), and ambulatory BP monitoring (ABPM); definitions of isolated systolic hypertension, white-coat, masked, and nocturnal hypertension.
  • Stratified BP Targets by Age/Frailty:
    • ≥65 years: initial target <150/90 mmHg; if tolerated, <140/90 mmHg.
    • ≥80 years (robust): start at ≥150/90 mmHg, target <150/90 mmHg; may titrate to <140/90 mmHg if well-tolerated.
    • Frail elderly ≥80 years: start at ≥160/90 mmHg, target SBP <150 mmHg (avoid <130 mmHg).
     
  • Pharmacologic Therapy:
    • Preferred first-line: low-dose thiazide diuretics, CCBs, ACEIs/ARBs.
    • ≥80 years/frail: low‑dose monotherapy as initial strategy (Class I, Level A).
    • Most others: initial low‑dose combination therapy (Class I, Level A).
    • Avoid dual RAS blockade (ACEI + ARB; Class III, Level A).
     
  • Special Populations: Frail elderly, those with orthostatic hypotension, cognitive impairment, polypharmacy, and comorbidities (CAD, CKD, stroke, diabetes).
  • Non-pharmacology: Salt restriction, physical activity, weight management, sleep, and psychosocial support.
  • Community & Integrated Care: Primary care, home monitoring, and geriatric multidisciplinary management.
