2021 Off-Label Use Recommendations for Intravenous Azithromycin in Children

 

1. Indications for Off-Label Use (超说明书适应症)

 
  • Severe community-acquired pneumonia (CAP) in children with
    • Atypical pathogens (Mycoplasma, Chlamydia, Legionella)
    • Mixed bacterial infection
    • Severe clinical manifestations or respiratory failure
     
  • Severe Mycoplasma pneumoniae pneumonia with
    • Lobar consolidation
    • Pleural effusion
    • Systemic inflammatory response
     
  • Other severe refractory infections caused by macrolide-sensitive pathogens
     
    (when oral administration is not feasible)
 

2. Dosage Regimens (给药方案)

 

General severe pneumonia

 
  • Loading dose: 10 mg/kg IV once daily on Day 1
  • Maintenance dose: 5 mg/kg IV once daily from Day 2
  • Duration: Total 3–5 days IV, then switch to oral step-down therapy
 

Severe/refractory Mycoplasma pneumonia

 
  • 10 mg/kg IV once daily for 3–5 days
  • May extend appropriately according to clinical response
  • Step down to oral macrolide after defervescence and improvement
 

3. Route & Administration (给药途径与配制)

 
  • Concentration: 1.0–2.0 mg/mL
  • Infusion duration: 1–3 hours
  • Avoid rapid infusion to reduce cardiovascular and gastrointestinal adverse events
 

4. Contraindications & Precautions (禁忌与慎用)

 
  • Hypersensitivity to macrolides or excipients
  • Caution in patients with
    • Severe hepatic impairment
    • Cardiac arrhythmia, prolonged QTc interval
    • Myasthenia gravis
    • Electrolyte disturbances (hypokalemia, hypomagnesemia)
     
 

5. Key Safety Monitoring (安全性监测要点)

 
  • ECG monitoring for QTc prolongation
  • Liver function at baseline and during treatment
  • Gastrointestinal symptoms (abdominal pain, vomiting, diarrhea)
  • Cardiac symptoms: palpitations, chest discomfort
